Our resident profile is as follows:

Our supported-living model empowers each young person to make better choices that result in excellent outcomes. We work with families, support workers, advocates, and those who are in charge of their care and support. We provide the following support to individuals:
- Assisting individuals in developing relationships and gaining access to desired community activities
- Encouraging them to discover their own talents and abilities
- Providing practical assistance such as personal care, cooking, and healthy living instruction and support
- Assisting individuals in setting and achieving objectives
- Assisting them in gaining access to information that can benefit them

Our referral process
- Make a referral to the ICRIT Healthcarereferrals team via phone or email.
- An assessment is arranged and undertaken via our management team.
- We provide feedback on whether our service can meet the individual’s needs.
- We create a care plan using information from the assessment.
- If admission is agreed upon and plans for transition are arranged with the referring team following confirmation of acceptance of placement.
